Healthcare Survey
Counselling Survey This survey is aimed at patients who receive counselling as part of their treatment regime. The questionnaire exemplifies support available for those undergoing breast cancer treatment and receiving counselling. The survey is constructed so that it can be adapted to suit different patients needs.
Existing Conditions This survey is aimed at patients who have existing conditions, which then affect additional surgical activity. The questionnaire focuses on the care provided before, during, and after the operation. For the purposes of this work the existing condition is diabetes.
Patient Experience - Survey This survey details the feedback needed on pre and post operative care for a patient receiving surgery on toenails. It includes questions on the quality of care and information. The pain management process is detailed, as well as, key issues such as the application of dressings and the process of patient self-care.
Cardiovascular Patient Feedback This questionnaire is designed to provide specific feedback on the quality of care you received as a patient with heart problems. The purpose of the survey is to improve the information, support, and treatment, patients with heart problems receive.
GP Practice Feedback This survey samples the general public about their experiences using the NHS and in particular the GP practice services. This is particularly aimed at the practice with which you are registered.
Infection Control Survey Ward staff can use this questionnaire to cross check the infection control regimes and to provide evidence at periodic intervals to support improvement.
Inpatient Feedback A sample questionnaire that can be extended to include feedback on admission procedures and personal detail which can be used to ensure a range of responses across age and ethnicity without identifying the individual. For the purposes of this survey, the feedback starts with the stay on the ward.
Referrals and Admissions Information about the process of referral and admissions, from the user's perspective, can be collected and analysed using this survey.
Home Care Service The Department of Health set Domiciliary Care National Minimum Standards on 1st April 2003. There is a requirement within these standards to continuously monitor the quality of care provided. A yearly survey is one way in which this can be done.
